Who carries First Choice Health Network
First Choice Health Network is a regional PPO network used by a number of employers and third-party administrators across the Intermountain West and Pacific Northwest.
Because First Choice is a network rather than a single insurance company, the name on your card is often your employer's plan or a claims administrator, with the First Choice logo somewhere on the back. That logo is what matters for finding an in-network specialist.

Diagnosis with spirometry, control plans, and biologic therapy for severe disease.

Allergy injections are given during walk-in shot hours. No appointment is needed for a shot, but plan to stay 30 minutes afterward.

First Choice Health Network questions we get
My card has two logos on it. Which one do I give you?
Give us both. One tells us which network applies and the other tells us where the claim goes. It is a common setup and not a problem.
Do I need a referral?
PPO networks generally let you see a specialist directly. Your employer's plan may still have its own rules, so we verify before your appointment.
What if First Choice is my secondary coverage?
Tell us about both plans when you book. Coordinating them correctly up front is much easier than fixing a claim afterward.
